Laura R Sangaré, Noel S Weiss, Paula E Brentlinger, Barbra A Richardson, Sarah G Staedke, Mpungu S Kiwuwa, Andy Stergachis Malaria Journal 2011, 10:152 (6 June 2011)
Abstract | Full text | PDF

Editor’s summary
Overuse of anti-malarial drugs, especially ones that are no longer recommended, undermines malaria control efforts by fueling the spread of drug resistance and delaying appropriate treatment of non-malarial febrile illnesses. Improved diagnostic capacity is essential to ultimately improving the management of malaria-like symptoms during pregnancy and appropriate use of currently available anti-malarials.

Despite fair knowledge of malaria in Swaziland, there is a need for improving the availability of information through the preferred community channels, such as the traditional community district meetings or tinkhundlas. There is documented evidence suggesting that as the level transmission and disease decreases, so does the perception about the importance of malaria control activities.

Editor’s summary
Malariologists tend to think that the pathophysiology of malaria is unique. This thought-provoking review shows that much can be learned from events described with other pathogens and that falciparum malaria is primarily an inflammatory cytokine-driven disease. In particular, the inability of mitochondria, through the effects of inflammatory cytokines, to utilise available oxygen may play a crucial role.

Limited available data suggest that artemisinins are effective and unlikely to be cause of foetal loss or abnormalities, when used in late pregnancy. However, one cannot rule out rare serious adverse events, even in 2nd and 3rd trimesters and there is not enough evidence to assess risk-benefits of artemisinin in early pregnancy. Methodologically rigorous, larger studies and post-marketing pharmacovigilance are urgently required.

The paper provides a qualitative analysis of the programmatic issues faced when implementing IPT during pregnancy, from both provider and user perspectives. Of particular interest is the Tanzanian experience of the specific link made between SP-IPT and Steven Johnson¿s Syndrome by pregnant women, which is unique and has been one of the most important barriers to the effective uptake of ITP, as a result of media spin. This theoretical and mathematical review provides a new look at the Ross-Macdonald models, the basis of malaria epidemiology. The models describe the processes that drive malaria transmission, mainly from an entomological perspective, and they are, therefore, particularly useful to evaluate intervention programmes that reduce transmission.

Access to life saving malaria treatment within 24 hours of symptom onset is still far below the RBM targets. Improved case management requires a strengthened health system including community health initiatives and working with non-public sector compartments of the health system. The paper reviews different aspects of provider performance in the different compartments: public sector, community health workers and the private sector.

This study highlights some of the assumptions commonly used when constructing mosquito-malaria models and presents a realistic model of Anopheles gambiae s.s. and Anopheles arabiensis, and their interaction. The authors make a case that this new mosquito model, OMaWa, may improve the understanding of vector dynamics, which in turn can be used to better understand the dynamics of malaria.

The authors used matched regression analyses to assess the association of vector control (ITNs and/or IRS) under routine programme conditions, using national household surveys.

The development of insecticide-treated durable wall lining for malaria control: insights from rural and urban populations in Angola and Nigeria
Louisa A Messenger, Nathan P Miller, Adedapo O Adeogun, Taiwo Awolola, Mark Rowland Malaria Journal 2012, 11:332 (18 September 2012)

Editor’s summary
The operational success of durable wall ling as a long-lasting insecticidal substrate will be dependent on a high level of user acceptability as households must maintain correctly installed material on their walls for several years. In this study, there was a dichotomy between rural and urban respondents. Rural participants favoured wall adornments and accepted DL because of its perceived decorative value and entomological efficacy. By contrast, urban households rejected the DL based upon objections to its aesthetics and installation feasibility.
