The cinchona alkaloids are the longest standing antimalarial drugs in clinical use, but their mode of action is still incompletely understood. This is the first study to provide comprehensive physico-chemical information regarding these alkaloids.The results strikingly demonstrate that the antimalarially inactive threo compounds are all considerably more hydrophilic than the active erythro compounds and that they are also very weak inhibitors of beta-haematin formation in contrast to the strongly active erythro compounds that inhibit beta-haematin formation better.

However, one cannot rule out rare serious adverse events, even in 2nd and 3rd trimesters and there is not enough evidence to assess risk-benefits of artemisinin in early pregnancy. Methodologically rigorous, larger studies and post-marketing pharmacovigilance are urgently required.

This retrospective study shows that Olyset nets are popular, durable and with a much longer insecticide persistence than ordinary polyester nets. ITN programmes in rural malaria-endemic areas have shown that the re-treatment of nets faces numerous logistic problems and that a switch to long-lasting insecticidal nets represents, therefore, a very attractive option.
