As
part of our ongoing efforts to better
understand the global malaria situation, we are surveying major bilateral and
multilateral funders on their funding of malaria related endeavors in the year
2000. The data will be compiled and
analyzed to better inform governments, funders, non-governmental organizations,
and others whose common goal is the global control of malaria.
Please
find the attached survey that asks details of your agency’s malaria control
funding. The survey is divided into
four major parts:

Analysis by Funding Recipient
You
have two options as to how to fill out the survey:
Option 1: Fill the survey out electronically on
your computer with Microsoft Word using the boxes embedded in the document ,
save it, and reattach it to a response e-mail (note some calculations will
automatically happen in this case.)
OR
Option 2: Print the survey in either HTML or Word format, complete it, and fax it to:

We
realize that this is a detailed survey and may require some effort to fully
complete. We also understand that many programs are not malaria specific, but
rather part of integrated health programs.
However, we ask that you please do your best to complete all possible
information as we believe it will help us better inform all participants in
global malaria control. Please estimate to the best of your
knowledge whenever necessary and indicate you have done so.
