Awareness of sign or symptom, but easily tolerated: Transient or mild discomfort;
no limitation in activity; no medical intervention/therapy required |

Moderate |
Discomfort enough to cause interference with usual activity: some assistance may be
needed; no or minimal medical intervention required |
| 3 |
Severe |
Incapacitating with inability to work or perform usual activity: some assistance usually
required; hospitalization possible |
| 4 |
Life-threatening |
Extreme limitation in activity, significant assistance required; significant medical
intervention/therapy required; hospitalization probable |
